How NeuroHaven handles the information you give us in the client intake questionnaire.
What we collect
The client intake form at neurohavenmiami.com/report collects your name, date of birth, and your answers to a standard brain-training questionnaire covering sleep, focus, mood, stress, and related areas. You choose what to share. Nothing is collected from you anywhere else on this site beyond ordinary, anonymous traffic statistics.
Why we collect it
Your answers are used to prepare your qEEG brain map and your training plan, and for no other purpose. They are not used for advertising, they are not sold, and they are not shared with any third party for marketing.
Where it goes
Your submission is sent over an encrypted connection and stored in a private, access-controlled database. Raphael Tordjman, the practitioner responsible for your training, is the only person who accesses it. The questionnaire file is then loaded into the analysis software used to build your plan.
Notifications
When you submit the form, your trainer receives a notification containing a summary and a secure, expiring link. The full questionnaire is not sent as an email attachment by default, because email is not an encrypted transport.
Your choices
You can ask what we hold about you, ask for a copy, or ask us to correct or delete it. Write to raphael@neurohavenmiami.com and we will respond. If a client is under 18, a parent or guardian should complete the form on their behalf.
How long we keep it
Records are kept for as long as needed to support your training and to meet the record-keeping obligations that apply to the practice. If you want your record removed sooner, contact us at the address above.
A note on what this is
NeuroHaven is a brain-training practice, not a medical clinic. Neurofeedback is a training method, not a treatment for any condition, and the questionnaire is not a diagnostic instrument.
